in order to gain access to the following programs:

Access to Recovery
Case Management
FACT Peel+
Treat at Home
During office hours, Monday to Friday, 9 a.m. – 4:30 p.m., an
intake worker can complete a form for you, your loved one, or
client, and make an appointment for an intake assessment at the
Resource Centre, 2 County Court Blvd., Unit 102, Brampton, ON.
Alternatively you may complete a form (click here) and fax it to
905-456-7492, after which an intake worker will call to make an
appointment.
