that provides comprehensive, community based treatment for people

individuals with severe mental illness
-

flexible, and comprehensive treatment, support and
rehabilitation
-

-
Skills teaching and assistance with daily living
activities
-

ACTT strives to lessen or eliminate the
debilitating symptoms of mental illness each individual client
experiences and to minimize or prevent recurrent acute episodes of
the illness, to meet basic needs and enhance quality of life, to
improve functioning in adult social and employment roles, to
enhance an individual's ability to live independently in his or
her own community, and to lessen the family's burden of providing
care.
-
Assertive Community Treatment uses a
multidisciplinary team approach to provide intensive services
where and when clients need them—in their homes, at work and in
other community settings—24 hours a day, seven days a week
-
Team members share responsibility for clients
served by the team
-
Team members encourage and support clients’
through regular conversations, to help them with their
understanding of their reluctance or hesitation to take their
medications
-
Team members provide clients with regular
reminders and supervision around taking medications over both
short and long periods of time, as needed e.g., phone calls,
daily medication drop-offs, individual medication packaging
ACTT services are individually tailored with each
client and address the preferences and identified goals of each
client. The approach with each client emphasizes
relationship-building and active involvement in assisting
individuals with serious mental illness to make improvements in
functioning to better manage symptoms, to achieve individual
goals, and to maintain optimism. The Canadian
Mental Health Association/Peel Branch is one of three providers in
the Peel Region of ACT teams. Working in partnership with these
providers to establish service catchment areas, CMHA’s team is
responsible for clients in the north-east part of Peel Region. In
the city of Brampton, the area is bounded by Hwy. 50 to the east,
Hwy. 410 to the west, just south of Hwy. 407 to the south and
Mayfield Road to the north. CMHA/Peel ACT also is responsible for
clients living in Bolton.
